Employer Groups of 50 and above: $10.95 PEPM with $0 added fees
Employer Groups of 10 to 49: $15.95 PEPM with $0 added fees
Employer Groups of less than 10 employees: Not available as a list bill. Click here to see our individual plan priced at $24.95 per month with a $25 consult fee per visit.
